Today the former Municipal Hospital of San Juan houses the Puerto Rico Art Museum. The original team who transformed the space was joined by architects Otto Reyes Casanova and Luis Gutiérrez Negrón, who established "The Museum Group" in order to develop the project. This group was responsible for the restoration and rehabilitation of the former Municipal Hospital of San Juan, the design of the new 10,000 square foot east wing, botanical garden, and more.

The historic building, originally designed during the 1920s by the architect William H. Shimmelphening, is the only existing building of the 13 that included the former Municipal Hospital of San Juan. The building operated as the hospital's surgery complex until 1966, the year in which the Medical Center of Río Piedras is inaugurated. Later it housed the facilities of the Department of Public Works until 1975.
